29 CFR  1910.147(c)(4)(i):  Procedures had not been developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about January 19, 2022, an employee was exposed to amputation hazards resulting in multiple amputations of the fingers of the left hand, while performing maintenance and servicing operations on a Sterling Extruder/Machine Roller, and the employer had not developed, documented, and utilized a machine specific energy control procedure for this piece of equipment.

29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures:  On or about January 19, 2022, an employee was exposed to amputation hazards resulting in multiple amputations of the fingers of the left hand, while preparing to perform maintenance and servicing operations on a Sterling Extruder/Machine Roller, and the employer had not provided sufficient authorized employee retraining to enable the employee to identify and control the pneumatic energy associated with the Sterling Extruder/Machine Roller after the employee was transferred from the South Plant to the North Plant and assigned to work on the Sterling Extruder/Machine Roller.

29 CFR  1910.213(g)(1):  Sliding cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw:  On or about January 20, 2022, an employee was exposed to amputation hazards while operating a De Walt saw without a guard/hood, which would drop on top and remain in contact with the material being cut.
