29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures: On or about January 19, 2022, an employee was exposed to amputation hazards resulting in multiple amputations of the fingers of the left hand, while preparing to perform maintenance and servicing operations on a Sterling Extruder/Machine Roller, and the employer had not provided sufficient authorized employee retraining to enable the employee to identify and control the pneumatic energy associated with the Sterling Extruder/Machine Roller after the employee was transferred from the South Plant to the North Plant and assigned to work on the Sterling Extruder/Machine Roller.
