29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about: January 21, 2022 Location: 38 Franklin Rd, Scarsdale, New York 10583  a) Employees were working on a inadequate 5ft by 12ft temporary working platform approximately 20ft high without fall protection installing fascia board. Employees were exposed to fall hazard.

29 CFR  1926.602(c)(1)(viii):A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck (C) Protection from falling objects as indicated necessary by the operating conditions.  On or about: January 21, 2022 Location: 38 Franklin Rd, Scarsdale, New York 10583  a) Employee was using a JLG1044C-54, series II rough terrain forklift to lift employees on a inadequate fabricated 5 ft by 12 ft temporary working platform to install fascia board. Unsecured platform fell with two employees causing injuries.
