29 CFR  1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  On or about April 20, 2022, the employer did not ensure all surfaces such as, but not limited to a respirator storage locker in the North Foundry and a table with the coffee maker in the South Foundry were maintained as free as practicable of lead surface contamination, exposing employees to the potential hazard of ingestion of lead.

29 CFR  1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of copper fume listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 0.1 milligrams per cubic meter of air:   On or about April 20, 2022, an employee  working in the South Foundry on HM1 was exposed to an 8-hour time weighted average (TWA) of 0.297 milligrams per cubic meter of air for copper fume, approximately 2.97 times the Permissible Exposure Limit (PEL) of 0.1 milligrams per cubic meter of air. The exposure was derived from a sample collected over a 432 minute sampling period, with zero exposure assumed for the unsampled period of 48 minutes.

29 CFR  1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  On or about April 20, 2022, the employer did not implement engineering and work practice controls to reduce and maintain exposure to copper fume to or below the PEL in the South Foundry while employees were working around HM1 during the production of copper alloys. An employee was exposed to airborne concentration of copper fume at an 8-hour time weighted average of 0.297 milligrams per cubic meter of air.
