29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Pioneer Enterprises LLC., doing business as, C&R Recyclers LLC, located at 19242 State Route 576 in Pioneer, Ohio:  On or about January 21, 2022 The employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees refurbishing propane tanks. Employees performing work activities on 500 gallon propane tanks without an employer assessment were exposed to eye, skin, and bodily injuries.

29 CFR  1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a) Pioneer Enterprises LLC., doing business as, C&R Recyclers LLC, located at 19242 State Route 576 in Pioneer, Ohio:  On or about January 21, 2022 the employer did not ensure that each powered industrial truck operator successfully completed the training and evaluation, specified by OSHA's PIV training standards prior to permitting employees to operate powered industrial trucks (forklifts). Employees operating or working near forklifts were exposed to struck-by and crushing injuries.

29 CFR  1910.307(c):Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  a) Pioneer Enterprises LLC., doing business as, C&R Recyclers LLC, located at 19242 State Route 576 in Pioneer, Ohio:  On or about January 21, 2022 the employer did not ensure equipment, wiring methods, and installations of equipment in class I division II locations were intrinsically safe, approved for the hazardous location, or safe for the hazardous class location. Employees were exposed to fire and explosion hazards in that electrical installations, electrical box, located within twenty feet of a propane tank containing pressurized propane liquid was not approved for the class I division II hazardous location.

Two recordable injuries that occurred on or about January 21, 2022 as a result of a fire, were not entered on the OSHA 300 Log (or equivalent) within seven calendar days.
