29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a. On or about 01/22/2022, at the jobsite - an employee conducting pressure washing from the roof of a public school was exposed to approximately 28 feet fall hazard, in that, fall protection was not installed or utilized.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a. On or about 01/22/2022, at the jobsite - an employee conducting pressure washing from the roof of a public school approximately 28 feet high was not provided with fall protection training.
