Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On 1/26/2022,  workers from Pablo Exteriors were observed working on the roof of two new residential buildings located at the 12 New Sugar Creek Road in Fenton, MO.  The employees were on top of a 6:12 slope roof and were not utilizing guardrail systems, a safety net system, or a personal fall arrest system, nor were employees provided with an alternative fall protection measures,  exposing them to fall to the ground below.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.   On 1/26/2022, workers from Pablo Exteriors installing new roofing material at a residential building located at the 12 New Sugar Creek Road in Fenton, MO, were using a ladder improperly.  The ladder did not extend three feet beyond the landing or edge of the roof.
