At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, in the roof trusses: On or about January 26, 2022, the employer exposed 2 framing employees to a 10 feet fall hazard in that they were not provided a ladder or stairs to access the web frames of the trusses.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, on the roof: On or about January 26, 2022, the employer exposed three framing employees to a 10-foot fall hazard at the roof access ladder from the ground to the roof in that the ladder rails did not extend 3 feet above the upper landing surface of the roof at the eave.  b.    At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, on the roof: On or about January 26, 2022, the employer exposed 2 employees to a 6 feet fall hazard in that the access ladder to the second-floor dormer roof did not extend above the upper landing surface.

At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, on the roof: On or about January 26, 2022, the employer exposed 2 framing employees to a 6 feet fall hazard in that a folded step ladder was leaned against the front dormer on the sloped lower roof for access to the dormer roof.

At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, in the roof trusses: On or about January 26, 2022, the employer exposed 2 framing employees to a 26 feet fall hazard down through the open trusses in that they were not protected by a fall protection system.  b.   At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, on the 7:12 pitch roof: On or about January 26, 2022, the employer exposed 2 framing employees to a 28 feet fall hazard down through open trusses and over the raked end of the roof in that they were not protected by a fall protection system.  c.   At the residential framing job site in the Highpoint subdivision of Rivertown, at 153 Woodbay Court, lot 73, in Saint Johns, Florida, on the 3:12 pitch roof: On or about January 26, 2022, the employer exposed 3 framing employees to a 10 feet fall hazard from the front eave in that they were not protected by a fall protection system.   Bahenas Multiservices, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1327153, Serious citation number 1, item number 2, and was affirmed as a final order on August 28, 2018, with respect to a workplace located at 14904-14900 Venosa Circle, Montevilla Subdivision, Jacksonville, Florida, 32246.
