Location: 6801 West Side Ave. North Bergen, NJ 07047 - Shipping Warehouse Dock 70 a) Employee exposed to fall and crushing hazards; employee was operating a forklift inside a cargo trailer when the truck driver began to drive away, exposing the forklift operator to roll backwards and fall out approximately 4 feet to the ground surface. The incident occurred on or about 12/27/21.
