29 CFR 1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the north side of the construction site, employees engaged in cornice work use pneumatic nail guns without wearing eye protection were exposed to struck-by hazards and eye injuries.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used. On January 27, 2022, employees performing cornice work utilized an extension cord with the outer sheathing split exposing internal wiring. The employees were exposed to electrical shock hazards.
