OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to smoke inhalation hazards and other hazards.  a) Worksite; Warehouse Waste Storage Room - On or about January 27, 2022, the sensor in the Waste Storage Room was not properly activating resulting in employees being locked in the room. Employees were unable to exit the room from inside when the door was closed.  b) Worksite; Mezzanine Room/Labeling Room - On or about March 24, 2022, the exit route leading into the Server Room required employees to pass through a locked door that had no emergency push bar. The door could only be unlocked with an electronic swipe card, and the employees working in the Mezzanine Room/Labeling Room were not provided with swipe cards to open the door. Employees were thus unable to exit the Mezzanine/Labeling Room.  c) Worksite; Mezzanine Room/Labeling Room - On or about March 24, 2022, employees were exposed to hazards of delayed egress in that the exit route through the Mezzanine Room/Labeling Room was blocked by a mobile stair ladder.  d) Worksite; Server Room - On or about March 24, 2022, employees were exposed to hazards of delayed egress in that the exit route through the Server Room was blocked by chairs and a desk.  e) Worksite, Mezzanine - On or about March 24, 2022, exit directional signs were not posted in the Mezzanine area to direct employees to the nearest exit.
