29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) HAL Department - On or about December 21, 2021,  the employer did not did not ensure that the roller with a blade attached to it was guarded on the Inter Folder #403 Paper Converting machine, model #1004295, to protect the employee's hand from the rotating blade.  An employee suffered an amputation of the tip of the right middle finger while webbing a roll of aluminum paper.
