29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about January 26, 2022, at the worksite located at N8 W22100 Johnson Drive, Waukesha, WI, the employer did not ensure adequate guarding was implemented on components of the Fahrut pita line machine to include, but not limited to, the sheeter units 1, 2, and 3, and the pre-forming station. Employees are exposed to in-running nip points from the rollers while pushing dough through the individual stations, thereby exposing workers to caught-by and struck-by hazards.
