29 CFR  1910.132(a):Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about January 20, 2202, the employer did not ensure protective equipment for the extremities, consisting of long-sleeved work shirt uniforms, was used properly to prevent employees from making physical contact with the moving and rotating parts of Sharp, Model 26260B Lathe Machine.  The employer did not ensure machinist employees, required to wear long-sleeved work shirt uniforms, wore them in a manner to prevent loose clothing and/or sleeves from contacting and being caught-in the moving and rotating parts of the machine.

29 CFR  1910.132(f)(1)(iii):How to properly don, doff, adjust, and wear PPE;  On or about January 20, 2202, the employer did not provide training to each employee about how to properly wear personal protective equipment (PPE).  The employer did not ensure machinist employees required to wear long-sleeved work shirt uniforms were trained to ensure they were worn properly to ensure no loose clothing and/or sleeves could be caught-in moving and rotating parts of machines or equipment.

29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about January 20, 2202, the employer did not verify through a written certification that a required workplace personal protective equipment (PPE) hazard assessment had been performed for employees required to utilize PPE while performing machining and other machine shop related work activities.
