29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employees were exposed to struck-by hazards. Employees engaged in powered industrial truck activities were exposed to struck-by hazards in that the employer utilized a front-end attachment to aid in material movement without prior manufacturer approval. Employees were utilizing a Arrow front end attachment shaft, model number 5060-5.5, length 5.5 feet on the LPG Nissan forklift, model number MCP1F2A25LV.

29 CFR 1910.219(c)(2)(i):All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: Employees were exposed to caught-in and amputation hazards. Employees engaged in production activities on machine 143 in the main production area in that an exposed powered shaft was not guarded. The exposed powered shaft is located 31 inches from the floor with a 36 inch walk way area which puts employees in close proximity of the hazard.
