(a) On or about January 31, 2022 and at times prior to that date, for the employees exposed to struck-by hazards where a Central Machinery 20 Ton H-Frame hydraulic press, Model Number 32879, Serial Number 352352030 was operated to press bearings in the maintenance shop located at 975 Kirman Avenue, Reno, Nevada.
