29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) 7175 Burnite Mill Road in Felton, DE 19943: On or about November 30, 2021, the employer failed to ensure subcontractor's employees conducting roofing operations work on an existing single family home while being unprotected in all directions, approximately 24 feet above grade, were provided adequate fall protection.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   a) 7175 Burnite Mill Road in Felton, DE 19943: On or about November 30, 2021, the employer failed to ensure subcontractor's employees used ladders that extended at least 3 feet above the upper landing to access the roof, exposing employees to fall hazards approximately 24 feet above grade.
