29 CFR  1910.36(d)(1):  Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge.  A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  (a)  On or about February 2, 2022 , doors marked as exits were opened with knobs instead of panic bars, such as but not limited to the two doors in shipping by the loading dock,  the door at the rear of the building in finishing, and the set of double doors in fabrication by the employee parking lot.

29 CFR  1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-2015, which is incorporated by reference as specified in Sec. 1910.6.   "All compressed gas cylinders in service or in storage at user locations shall be secured to prevent falling or rolling. "   (a)  On or about February 2, 2022, a cylinder of propylene was not secured to prevent falling and rolling, in the area of the welding cart.

29 CFR  1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  (a)  The employer had not completed the required certification that a workplace hazard assessment had been performed.
