29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were deenergized before employees began to work on or near live parts. On or about January 19, 2022, and at times prior thereto, at building 6325, an employee was installing a circuit breaker into a circuit breaker panel and did not ensure the electricity to the circuit breaker panel was deenergized. This exposed the employee to an arc flash hazard.
