29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a)Coatesville Veterans Affairs Medical Center located at 1400 Blackhorse Hill Road Coatesville, PA: On or about December 29, 2021, an employee was hospitalized of COVID-19. The employer was made aware of the hospitalization on December 30, 2021 and did not report the work-related hospitalization to OSHA within 24 hours. The employer reported the hospitalization to OSHA on February 1, 2022 when notified that the employee was readmitted to the hospital.

29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Coatesville Veterans Affairs Medical Center located at 1400 Blackhorse Hill Road Coatesville, PA: On or about December 29, 2021, an employee was hospitalized as a result of a confirmed work-related exposure to COVID-19. The employer did not record the work-related illness of the employee on the OSHA 300 log or its equivalent. Condition was noted on or about February 1, 2022.
