29 CFR  1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  On or about January 26, 2022, at an establishment located at 1016 School Street in Two Rivers, Wisconsin, in the Card Machine Department, the Card Stripping/Cleaning Procedure used for the Davis & Furber #11 Card Machine did not provide specific procedural steps for shutting down or removal of belts for the gear reducer while employees used a hand wheel during the stripping and cleaning activities exposing employees to in-running nip points.  The Miller Waste Mills Inc. dba RTP Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1282618, citation number 1, item number 1a and was affirmed as a final order on March 18, 2018, with respect to a workplace located at 1016 School Street, in Two Rivers, Wisconsin.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about January 26, 2022, at an establishment located at 1016 School Street in Two Rivers, Wisconsin, in the Card Machine Department, employees were exposed to machine hazards associated with moving parts when they engaged in stripping activities on the Davis & Furber #11.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to manually turn the hand gear located next to the rotating belt reducer.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.  The Miller Waste Mills Inc. dba RTP Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1282618, citation number 1, item number 1b and was affirmed as a final order on March 18, 2018, with respect to a workplace located at 1016 School Street, in Two Rivers, Wisconsin.
