29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted:  a) Northern Onondaga Volunteer Ambulance Inc, located at 4425 Buckley Road, Liverpool, NY 13090, on or about 2/15/2022 and thereafter: The employer did not provide an annual fit test to all employees required to wear N95 and/or MSA Advantage 200 LS filtering facepiece respirators for employees including but not limited to paramedics; emergency medical technicians and critical care technicians while providing patient care that exhibit cold/flu symptoms. Last fit tests were conducted in November 2020.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  a)  Northern Onondaga Volunteer Ambulance Inc, located at 4425 Buckley Road, Liverpool, NY 13090, on or about 2/15/2022:  A recordable injury occurred on 01/30/2022 when an employee received a prescription antibiotic due to a dog bite on employees left thigh. The case was not recorded on the 2022 OSHA 300 Log.
