On or about February 02, 2022, at a residential construction site located on 7534 Townsend Bunting in Katy, TX, employees were exposed to fall hazards greater than 6 feet when performing roofing work on the one-story house without a fall protection system.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about February 02, 2022, at a residential construction site located on 7534 Townsend Bunting in Katy, TX, employees were exposed to a fall hazard when using an extension ladder to access the roof without the side rails at least three feet above the roof landing, at the following locations: a. on the east side b. on the west side
