The employer did not ensure adequate machine guarding was provided to prevent employees from entering or inserting body parts into machines or equipment.  Employees engaged in the manufacture of products worked at or around equipment that included, but was not limited to Bosh 11 Machine, were able to change the wrapper in the machine while the machine was running as the interlocks were removed, so the equipment did not have a physical guard and/or interlocks to stop the equipment when the guard was opened.   Violation occurred on or about January 21, 2022.
