29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows: a) On or about February 1, 2022, at the commercial construction site, employees were exposed to fall hazards of more than 10-feet above the ground level below while working from a tubular welded frame scaffold that was not fully planked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews: a) On or about February 1, 2022, at the commercial construction site, employees were exposed to fall hazards of more than 10-feet while working from a tubular welded frame scaffold that was not equipped with a guardrail system or personal fall arrest system.
