OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to potential fire, explosion, and burn hazards associated with a furnace door not being seated properly:   a.) MacLean-Fogg Saegertown, LLC; heat treat - On or about, 1/22/22, and at times prior to, employees were operating the heat treat furnace while the quench vestibule door was not seated properly which allowed flames to periodically and unexpectedly escape at the bottom of the door.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a.) MacLean-Fogg Saegertown, LLC; heat treat - On or about, 1/22/22, and at times prior to, employees were operating the heat treat furnace while the quench vestibule door was not seated properly which allowed flames to periodically and unexpectedly escape at the bottom of the door. Employees were not required to wear Fire Resistant Clothing (FRC) while working near the furnace door.  Appropriate PPE would include, but not be limited to, jackets, gloves, eye and face protection.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
