OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees by exposing employees to struck-by hazards: (a) On or about 1/28/2022 and at times prior to at 3200 Rodeo Court, Bessemer, AL 35022, employees were exposed to struck-by hazards at lanes #79, 87, 88, 93, 94, and 96 where broken overhead garage doors weighing approximately 364 pounds were not maintained in a safe working condition.

29 CFR 1910.26(d): The employer did not ensure that measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semitrailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard: (a) On or about 1/28/2022 and at times prior to at the van lines, employees were exposed to fall hazards due to gaps of two feet between the van and the dockboard when packages were being loaded.
