In the Distribution Warehouse: Rack BE2 used to store pallets of boxed soft drink concentrate had a damaged column.  Employees were exposed to struck-by and crushed-by hazards.  b. In the Distribution Warehouse: Rack BE9 used to store pallets of bottled soft drinks  had a damaged column.  Employees were exposed to struck-by and crushed-by hazards.

In the Warehouse:  A GP CPD15 high lift forklift truck was not equipped with a functional operator restraint system.  Employees were exposed to crushed-by hazards and struck-by hazards.  b. In the Ice Plant: A Clark C2OSC high lift forklift truck was not equipped with an operator restraint system.  Employees were exposed to crushed-by hazards and struck-by hazards.
