29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about February 2, 2022, at the "L" bender tube bending area: (a) the electronic presence sensing device (PSD) did not extend to the full length, width and depth of the machine, allowing employees access to the equipment without the PSD engaging to prevent activation of the controls.  This condition exposed employees to caught in and crush hazards.

29 CFR  1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  On or about 2/09/2022 employees were exposed to caught in and crushing hazards at the "L" bender in the tube bending area of the facility in the following instances: a) where there was a gap of approximately 12 inches between the security gate and the north wall at the north end of the barrier fence on the  east side of the bender; b) the security gate located at the north end of the east barrier fence was not secured to prevent entry during the operation of the bender.
