The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. 29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. On or about January 31, 2022, employees conducting carpentry operations from a wooden platform that was elevated by a Pettibone B-66 Telehandler were exposed to fall and crushing hazards. The rough terrain forklift was not designed to be used for elevating personnel. No abatement certification or documentation is required for this item.
