29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Montana Tile & Stone Designers LLC, Shop: On or about February 3, 2021 and at times prior, employees were exposed to unexpected energization of equipment while conducting maintenance and changing out saw blades on the CMS SPRINT saw. The employer failed to develop, document, and utilize procedures to effectively control hazardous energy for employees that perform maintenance and saw blade changes.

29 CFR 1910.305(j)(2)(vii):A receptacle installed in a wet location where the product intended to be plugged into it was to be attended while in use (for example, portable tools) did not have an enclosure that was weatherproof when the attachment plug cap was removed: a) Montana Tile & Stone Designers LLC , Shop: On or about February 3, 2022 and at times prior, employees were exposed to electrical shock hazards while working in and around the uncovered electrical receptacle located at floor level on the Northeast wall of the shop. b) Montana Tile & Stone Designers LLC, Shop: On or about February 3, 2022 and at times prior, employees were exposed to electrical shock hazards while working in and around the uncovered electrical switches located near the unmarked exit door on the Southwest wall of the shop. c) Montana Tile & Stone Designers LLC, Shop: On or about February 3, 2022 and at times prior, employees were exposed to electrical shock hazards while working in and around the uncovered electrical receptacle located in the middle of the Southwest wall of the shop.
