29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On the north side of the construction site, employees engaged in decking activities without fall protection were exposed to unprotected fall hazards of approximately 9 feet to the ground below.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On the north side of the construction site, the employer did not provide an extension ladder where employees could access and egress the residential building upper levels, exposing them to potential fall hazards.
