29 CFR 1904.39(a)(2): Basic Requirement. Employer did not within twenty-four (24) hours after an in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. At 8464 Chapin Industrial Drive on or around January 7, 2022 an employee of Trueblue Inc dba Staffing Management SMX fell and broke their hip requiring hospitalization in a work related incident and failed to report the incident within 24 hours.
