29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about February 8, 2022, at the above addressed jobsite, employees working in a trench were not wearing hard hats to protect against falling materials and debris.

29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a. On or about February 8, 2022, at the above addressed jobsite, employees were conducting sewer operations in a trench measured at eight (8) feet in depth.  The trench was not provided with cave-in protection, thereby exposing the employees to cave-in hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about February 8, 2022, at the above addressed jobsite, the portable ladder used to access the trench did not extend at least three (3') feet above the ground surface, thereby exposing employees to the hazards associated with falls.
