29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Instance A: On or about February 8, 2022, employees at Standard Technologies, LLC were exposed to struck by, caught in or between, and crushing injuries from pneumatic and electrical energy sources when performing maintenance activities and service within the Viking Blast and Wash System Model MR8068/VK8 "Shot Blaster" Machine. Instance B: On or about February 8, 2022, employees at Standard Technologies, LLC were exposed to point of operation nip points, amputation, and laceration injuries, and struck by injuries from hydraulic and electrical energy sources when performing maintenance activities to the Press Brake #1 - Cincinnati Incorporated (S/N 41920). Instance C: On or about February 8, 2022, employees at Standard Technologies, LLC were exposed to point of operation nip points, amputation, and laceration injuries, and struck by injuries from hydraulic and electrical energy sources when performing maintenance activities to the Press Brake/Hydraulic Sheer #2 - Cincinnati Incorporated (S/N 46252).

The employer did not ensure that defects when found were immediately reported and corrected.
