At 3261 Dillon Street, in Jacksonville, Florida 32254: On or about February 3, 2022, the employer exposed employees to head  injuries, in that protective helmets were not utilized by employees working at ground level and beneath other employees working from the roof area.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 4:12 pitched roof, on a residence located at 3261 Dillon Street, Jacksonville, Florida 32254: On or about February 3, 2022, the employer exposed employees to fall hazards ranging from 9 to 17-foot, in that fall protection was not utilized by employees while installing roof shingles.
