On or about Wednesday, February 2, 2022 at 222-40 96th Avenue, Queens Village, NY, 11429 - Press Room  Employee using a two hand punch press to preform metal stamping  activities without a  guard.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.1.
