29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a. In the Maintenance Office:  A relocatable power tap designed for low amperage applications was used to power a microwave which was plugged in, in series "daisy chained" through an extension cord. Employees were exposed to fire hazards.  b. In the Maintenance Office:  A metallic outlet box designed to be installed in a wall was used as the receptacle end of an extension cord to power a microwave. Employees were exposed to fire and electrical shock hazards.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:   a. In the Engineering/Electrical Room:  Portable fire extinguishers were not visually inspected at least monthly. Employees were exposed to fire hazards.   b. In the Main Kitchen:  Portable fire extinguishers were not visually inspected at least monthly. Employees were exposed to fire hazards.   c. In the Banquet Kitchen:  Portable fire extinguishers were not visually inspected at least monthly. Employees were exposed to fire hazards.  Lotte Hotel Guam was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 1273556, citation number 2, item number 5 and was affirmed as a final order on February 26, 2018 with respect to a workplace located at 185 Gun Beach Road, Tamuning, Guam.

Employees were exposed to fire hazards.
