On or about February 8, 2022, and at time prior thereto, the walking-working surfaces around the storage facility were not keeping free from water leaks and spills. This exposed employees to a slip and trip hazards associated with multiple uneven surfaces covered with pools of stagnant water.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about February 8, 2022, and at time prior thereto, the employer did not maintain and implement the Hazard Communication program, which would address labeling, and other forms of warning on chemicals containers, Safety Data Sheets and employees' information and training on the hazards associated with chemicals used at this site. Employees were involved in assembling, maintaining and cleaning disposal and rental services of equipment, tanks and portable potty/restrooms units and were exposed to hazardous chemicals and materials.
