a) NBN Powder Packaging, LLC located at 955 Taylor Street in Elyria, Ohio: On or about February 8, 2022, the employer did not provide employees with effective training on how improper fit, usage or maintenance can compromise the protective effects, how to inspect the respirator, put on and remove the respirator, check the seal of the respirator, procedure for storage of the respirator, and how to recognize medical signs and symptoms preventing effective use of respirators. The employer required employees to wear half faced elastomeric respirators and N95 filtering facepiece respirators while weighing chemical powders. Employees were exposed to hazards associated with particulates not otherwise recognized such as but not limit to, respiratory tract irritation.
