29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a) NBN Powder Packaging, LLC located at 955 Taylor Street in Elyria, Ohio:  On or about February 8, 2022 the employer did not verify through written certification that a workplace hazard assessment was performed identifying the workplace evaluated, the person certifying the evaluation, and the date of completion prior to employees engaging in the weighing dry chemical powders. Employees were exposed to hazards associated dry chemicals powders handled in the facility such as, but not limited to, skin, eye, and respiratory tract irritation.

29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) NBN Powder Packaging, LLC located at 955 Taylor Street in Elyria, Ohio:  On or about February 8, 2022 the employer did not ensure that a written respiratory protection program with worksite specific procedures was implemented to protect employees from respiratory hazards. Employees engaged in the weighing of various chemical powders were required to don filtering face piece respirators or elastomeric half mask respirators pending  which powders are being handled.

a) NBN Powder Packaging, LLC located at 955 Taylor Street in Elyria, Ohio:  On or about February 8, 2022, the employer did not provide employees with effective training on how improper fit, usage or maintenance can compromise the protective effects, how to inspect the respirator, put on and remove the respirator, check the seal of the respirator, procedure for storage of the respirator, and how to recognize medical signs and symptoms preventing effective use of respirators. The employer required employees to wear half faced elastomeric respirators and N95 filtering facepiece respirators while weighing chemical powders. Employees were exposed to hazards associated with particulates not otherwise recognized such as but not limit to, respiratory tract irritation.

Employees were exposed struck by hazards in that forklift operators did not receive adequate training in forklift operations.
