29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): a) 107 Pierce Rd #1310, Clifton Park, NY - On or about February 02, 2022, at the main manufacturing facility, an employee was exposed to an amputation hazard while operating an aluminum cut off saw. The saw did not have a warning sign identifying hazard areas on the machine where a disk blade exposed employees to a laceration/amputation hazard.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) 107 Pierce Rd #1310, Clifton Park, NY-On or about February 02, 2022, at the main manufacturing facility an employee was exposed to an amputation hazard while operating an aluminum cut off saw. The saw was inadequately guarded at side points of the machine where a disk blade caused a laceration/amputation hazard.
