29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about February 9, 2022 and at times prior, at the screening room, the shaker platform had unprotected sides, no guardrails, no safety nets, and no personal fall protection was used to protect an employee from falls greater than 4 feet to a lower level.

On or about February 9, 2022, and at times prior, at the oven-tumbler, a sprocket wheel and chain were less than 7 feet above the floor and were not enclosed.  This exposed employees to a caught in hazard.  b.  On or about February 9, 2022, and at times prior, at the screening room, a sprocket wheel and chain for an auger under a platform was less than 7 feet above the floor and was not enclosed.  This exposed employees to a caught in hazard.

Employees recycled brown aluminum oxide that contained hazards chemicals including, but not limited to aluminum oxide (irritant), titanium dioxide (irritant, suspect carcinogen), and nickel (irritant, skin sensitizer, suspect carcinogen).  This exposed employees to the hazards of chemicals.
