29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces: At the facility, on or about 2/8/2022 employees entered and exited the rear of the hay trailer located outside at the edge of the pavement on the left side of the building. Employees were exposed to the hazard of falling as they climbed into and out of the trailer.

The log was not completed in the detail required in that the injury was not included in the description.
