Employees operated equipment that included, but was not limited to a Toyota, model 8FGCU2, Propane Forklift that had defective/damaged tires, causing tires to lose traction, exposing employees to tip-over hazards. Defects when found were not immediately reported and corrected: a. 300 Port Street Newark, NJ The employer did not ensure that powered industrial trucks were inspected for defects prior t use. Employees used equipment that included, but was not limited to, a Toyota, Model 8FGCU25 forklift that was not inspected prior to use, as tires on the forklift were damaged, to include tires missing chunks, tires having radial cracks, and splits on the tire sidewall were present. Violation Occurred on about 02/08/2022
