29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about February 9, 2022, three employees using extension ladders placed with their side rails near the roofline were exposed to a fall hazard of approximately nine feet as follows: (a) A ladder placed on the left side of the house/South-East; (b) A ladder placed at the front of the house/North-East; (c) A ladder placed on the right side of the house/North-West; (d) A ladder placed at the back of the house/South/West.
