29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  At the jobsite, on the roof, on or about 2/09/2022: Employee(s)were tearing off old roofing material and preparing to install new roofing material onto an estimated 8:12 pitch roof with a ground to eave height in excess of 10' without fall protection of any kind.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  (a)    At the jobsite, 119 Peirson Ave. in Newark, on or about 2/9/22: Lakeside Roofing & Contracting LLC workers were working on the roof without having been trained in regard to fall hazards and the recognition of them.          The employer does not have a written certification record verifying workers received fall protection training.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  (a)At the jobsite, on or about 2/09/22: Employees were using a ladder to carry tools and access the roof of the residential house and that ladder was not extended 3 feet above that roof.
