29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a) Marine Tech Wire & Cable, 631 South Richland Avenue, York, Pennsylvania - Employees did servicing and maintenance work, such as cleaning and replacing components, on machinery and equipment, to include the Wardwell 16-carrier braiders.  The written lockout procedure specified locking out the Wardwell 16-carrier braiders at the machine; however, not every braider, to include machines 19 through 25, had an energy isolating device at the machine capable of accepting a lockout device, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts and contact with electric current, on or about April 14, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Wardwell 16-Carrier Braiding Machines 1 through 34 - The spinning head and bobbins on each machine did not have guarding, exposing employees to struck-by, caught-in, and entanglement hazards, on or about February 9, 2022.  (b) Wardwell 16-Carrier Braiding Machines - On machines 1, 2, 4, 9, 10, 16, 19, 20, 21, 22, 24, 25, 29, 33, and 34, the footage counter gear did not have any guarding, and on machines 3, 5, 6, 7, 8, 9, 11, 12, 13, 14, 15, 17, 18, 26, 27, 28, 30, 31, and 32, the footage counter gear had partial guarding, at the front side of the machine where the gear met the capstan, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022.  (c) Wardwell 24-Carrier Braiding Machines - The spinning head and bobbins on each machine did not have guarding, exposing employees to struck-by, caught-in, and entanglement hazards, on or about February 9, 2022.  (d) Wardwell 24-Carrier Braiding Machines - The rotating guide wheel at foot level, under the four machines on the raised platform, did not have guarding, exposing employees to a caught-in hazard, on or about February 9, 2022.  (e) Wardwell 24-Carrier Braiding Machines - The footage counter gear on each of the three machines located in the center of the group of five Wardwell 24-carrier braiders did not have any guarding, and the footage counter gear on the left-most machine on the raised platform had a band guard, but the band guard did not have flanges extended inward beyond the root of the teeth on the exposed sides of the gear, exposing employees to caught-in and entanglement hazards, on or about February 9, 2022.  (f) United States Machinery Planetary Cabler - The three rotating discs, holding the pay-off spools, and the three rotating stranders had partial guarding, consisting of expanded metal from waist to shoulder height on the aisleway side of the discs and extending about half the length of the strander barrels, exposing employees to struck-by, caught-in, and entanglement hazards in the unguarded areas, on or about February 9, 2022.  (g) United States Machinery Planetary Cabler - The rotating taper did not have guarding over the side where the cable exits, exposing employees to caught-in and entanglement hazards, on or about February 9, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

(b) Wardwell 16-Carrier Braiding Machines - On machines 1 through 34, the worm gear at the rear of the machine, did not have guarding, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022.  (c) Wardwell 16-Carrier Braiding Machines - The bevel gears that ran the pitch gears did not have guarding by a complete enclosure in that on machines 1 through 11, 13 through 22, 24 through 28, and 30 through 32, the bevel gears did not have guarding on the left side (facing the machine), and on machines 12, 29, 33, and 34, the bevel gears did not have any guarding, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022.  (d) Wardwell 16-Carrier Braiding Machines - On machines 1 through 34, the spoked gear that ran the pay-off and take-up at the front of the machines did not have guarding, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022. 29 CFR  1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   (a) Wardwell 16-Carrier Braiding Machines 1 through 34  - The motor pulleys, with bottom height ranging from 13.5 inches to 23 inches above the floor, and head pulleys, ranging from 42.5 to 48 inches above the floor, at the right side of each Wardwell 16-carrier braider, did not have complete guarding in that the existing guards did not cover the pulleys on the inner sides nor on the head pulleys' periphery near the clutch lever on each machine.  In addition, on machines 10, 18, 19, 20, 21, and 22, the existing guards did not cover the motor pulleys on the outer sides, and on machines 11, 12, 14, 15, 29, 33, and 34, the existing guards did not cover the periphery of the motor nor head pulleys, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022.  (b) Wardwell 16-Carrier Braiding Machines 2, 8, 10, 16, 17, 18, 19, 20, 21, and 25 - The pulleys connected to the take up spool on each machine did not have adequate guarding, in that the top pulley, at waist height on each machine, had guarding that did not cover the inner side and back periphery of the pulley, and the bottom pulley, at knee height on each machine, had no guarding, exposing employees to caught-in and entanglement hazards, on or about April 14, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
