29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: 3730 N. Loop 1604 E.: On or about February 9, 2022, an employee was exposed to a fall hazard, while working from the platform of an aerial lift, without being effectively trained to recognize and avoid the hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): 3730 N. Loop 1604 E.: On or about February 9, 2022, an employee was exposed to a fall hazard while performing work from the basket of an aerial lift, without using fall protection.
