29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment:  a) On or about 02/09/2022, at the Buckeye Texas Processing (BTP) facility in Corpus Christi Texas, the employer failed to ensure that written procedures for testing or inspecting of the emergency shutdown button "Unit 100 ESD" (or tag 100-ESD-0133) were established.   b) On or about 02/09/2022, at the Buckeye Texas Processing (BTP) facility in Corpus Christi Texas, the employer failed to ensure that written procedures for testing or inspecting of process equipment were established  for the following devices :     I. Level Control LIC- 0405 (P&ID 100-PID-104 No.10)    II. Level Indicator LI-0413 (P&ID 100-PID-104 No.10)    III. Pressure Transmitter PT-1228 (P&ID 100-PID-112 No.5)    IV. Interlock I-10 (P&ID 100-PID-111 No.7)    V. Temperature indicator/control TI/TIC-1305 (P&ID 100-PID-113 No.12)    VI. Level indicator LI-1328 (P&ID 100-PID-113 No.12)

NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.  a)  On or about 02/09/2022, at the Buckeye Texas Processing (BTP) facility in Corpus Christi Texas, the employer did not inform employees by posting danger signs or by any other equally effective means on workplace contains permit spaces for the Main Crude Fractionator, 200-T-101, skirt  and the Side Strippers, 200-T-102 (Heavy Naphtha), 200-T-103 (Kerosene) and 200-T-104 (Gas Oils) skirt (These strippers are stacked on a top each other to form one tower).
