29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about February 10, 2022, employees on the West side of the site were performing roofing work on a building without fall protection at heights six (6) feet or more above the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about February 10, 2022 westside of the site, employees were performing roofing work while using an extension ladder that side rails did not extend at least three (3) feet above the upper landing of the surface being accessed.
