On or about February 10, 2022 at job site located in 617 Berkley Ave., Elmhurst, IL 60126, The employer did not ensure that employee working inside an excavation at approximately 12 feet in depth was using head protection. Employee was exposed to struck-by hazards from debris and employees working on the ground level when the employer failed to ensure a hardhat was utilized. The employer failed to ensure cave-in protection was utilized. b) On or about February 10, 2022 at job site located in 617 Berkley Ave., Elmhurst, IL 60126, the top of the trench box used was positioned below ground level, exposing an employee working in the trench to cave-in hazards from overtop of the box. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Additional shield system was not utilized. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
